Chronic Disease Management

The Melbourne General Practice Network’s Chronic Disease Management Program aims to provide a sustainable systems approach to strengthening GP involvment in chronic care and promoting best practice prevention, diagnosis and management of chronic conditions. MGPN’s Chronic Disease Management Program is based on the Wagner Chronic Care Model and encompasses the National Chronic Disease Strategy set by the Australian Government, and will provideinformation, training, education and resources to all practices within the MGPN in addition to providing individualised in-practice support to meet the specific needs of a practice.
