The Vic DOH and Australian Disease Management Assoc(ADMA) launched the Online Integrated Chronic Disease Management Clearinghouse last September (4/9/9). The Clearinghouse can be accessed at
www.adma.org.au and allows chronic disease management programs to upload any of their resources (eg. assessment tools, patient education material etc.) to the website so that others may access it. Similarly programs can download other programs' material and avoid duplication.
The clearinghouse is now being evaluated in order to assess its usefulness to disease management programs and would appreciate 5 minutes of your time to answer our survey.
